NEW STUDENT ENROLLMENT
New Student Enrollment Forms
Students who did not attend Checotah Public Schools last year will enroll Monday, August 4th at the Checotah High School Event Center from 12:00-8:00. Please remember to bring the following items for enrollment of your child: A birth certificate, immunization records, any legal documents (ie. child custody), social security numbers, CDIB and CITZ cards (If child is of Indian heritage), and Sooner Care Card (if applicable). If the family is receiving any assistance from a State or Federal Agency (TANF, SNAP, ETC.), please bring your card with you.
RETURNING STUDENTS ENROLLMENT
Returning Student Enrollment Forms
Students who attended Checotah Public Schools last year will enroll Tuesday, August 5th and Thursday, August 7th at the Checotah High School Event Center from 12:00-8:00. If the family is receiving any assistance from a State or Federal Agency (TANF, SNAP, ETC.), please bring your card with you.
We will have staff on hand to answer any questions parents may have concerning: transportation, immunizations, Indian Ed/JOM program, Special Education services, child nutrition, and technical support for parent log in to Student Gradebook. We are pleased to announce that on Tuesday and Thursday, staff from the McIntosh County Health Department will be there to give students any immunizations that are due.
All enrollment forms are available by clicking on the links above. To save time, parents may print the forms, fill them out and bring them enrollment day.
ALL PAPERWORK MUST BE FILLED OUT BY THE PARENT OR GUARDIAN.
